From symptom management and advance care planning, to creative and complementary therapies, our Wellbeing Hub is designed to help you live well with your life-limiting illness.
Whether it’s a drop in to discuss your concerns, attending an advice clinic or signing up to a wellbeing session to help reduce stress or alleviate symptoms, the Wellbeing Hwb is an inclusive space to support all adults living with a life-limiting illness in Central North Wales.

Frequently Asked Questions
What happens after I am referred?
Once a professional or self-referral has been received you will be offered an appointment to meet with one of the healthcare professionals at the Wellbeing Hub. In this appointment the team will discuss your condition with you, your needs, and what you are hoping to achieve through attending the Wellbeing Hub. They will help you to come up with a set of personal goals and agree a timetable to suit you. This will enable you to access the variety of services (in the Wellbeing Hub and other community services) which will help you to reach your goals. We will review your plan with you on a regular basis to ensure you are getting the right care and support.
Who are the Wellbeing Team?
Our specialist team of: Doctors, Nurses, Occupational Therapist, Physiotherapist, Admiral Nurse, Counsellor, Social Worker and Spiritual Lead, provide specialist palliative and social support to patients with life limiting illnesses and their families. We offer symptom management, therapeutic interventions and a social environment to enhance quality of life and independence.
Can my family attend?
Patients and families can choose to attend individual group sessions or attend for a full day where lunch is provided. At St Kentigern Hospice we are led by our patients and will provide tailored services to suit you.

Every year we care for approximately 500 people with life-limiting illnesses across central North Wales. We not only support our patients, but also their families and carers.
We tailor our care to meet the individual needs of every patient, improving their quality of life and promoting dignity.
Although our care is given freely, it is not without cost. Every donation, however small or large makes a positive difference.
